Jada Brooke was born to the parents of Darryl A. Van Hook and Jacquelyn Tobin.
A lifelong resident of Sidney,Ohio. She is survived by her daughter Everly.Three sisters. Jaylee, Chole, and Bianca. Aunts Brittany, Alexis, and Courtney. Grandparents Brenda and Aaron. And other close family and friends.
She was preceded in death by her grandpa Darryl K. Van Hook.
A memorial service will be held at Salm-Mcgill & Tangerman Funeral Home on Monday June 23, 2025 from 5pm-7pm.
